Leaving hospital – how do i arrange for my care services to re-start?
If you were receiving services such as home care before you went in to Hospital, and the level of support you need has not changed whilst you are in hospital, the ward staff will need to contact Integrated Adult Services Directorate duty desk on (01685) 724500 to advise them of your discharge date (date you are going home from Hospital) to re-start the package of care for you. Alternately you can contact them yourselves.
Leaving hospital – what support can i have or, how can i change the level of care i need?
If however you were not receiving services before being admitted to hospital or your needs have changed during your hospitalisation and you require a change to the amount of care you will need at home, a referral will need to be made to Integrated Adult Services Directorate duty desk (01685) 724507 with your consent. Following receipt arrangements will be made to visit you in hospital to assess the level of support you need.
Returning home - services provided
The services provided to support individuals to return home are:-
- Continuing Social Care (Home Care) - which provides support with personal care. Please click here for further information about this service.
- Community Occupational Therapy - provides aids and adaptations to aid independence.
- Initial Response Team - provides packages of care to individuals receiving support for the first time. This team works with persons in their own home to regain their independence for up to 6-weeks.
- Reablement Team - provides therapy support and programmes to individuals to make the most of their independence. This is made up of the following therapies: Physiotherapy; Occupational Therapy and Speech & Language Therapy.
