Initial Response and Reablement Service

The Initial Response and Reablement Service work with people to maximise their wellbeing and improve independence levels in daily activities following deterioration in abilities. The aim is to support people to remain at home for as long as possible. The teams work side by side each other to make sure that the right service is provided, at the right time to meet your particular needs.

The Reablement Team provides intensive, time limited therapy input to customers in their own home, which may include input from physiotherapy and occupational therapy.

The therapists and support staff will work with you to identify ‘what matters to you’ and support you in achieving goals related to daily activities. For example, increasing your independence with tasks such as dressing, cooking, shopping and moving around in and out of the house

Whether it is the first time you are needing support at home or following discharge from hospital, the Initial Response Service will support you for up to 6 weeks

The service will help you with personal care tasks such as washing and dressing, toileting, help with managing your medication as part of a package of care & support and meal preparation. Staff will work with you to find a way that will assist you in being as independent as possible.

We value your opinion of the service and you will have the opportunity to voice any concerns you have whilst your service is being provided.

During the first 6 weeks of receiving a service it will be reviewed. The purpose of the review period is to assess levels of need as you may improve during this time with the support of the enabling services.

If you need an ongoing service after this period, your service will transfer to a long term provider
