3. The total number of days taught by supply staff / cover supervisors / HLTAs.

4. The percentage number of external supply staff used that did not have a Qualified Teaching Status (QTS) qualification.

5. The percentage number of external supply staff used that were qualified teachers with QTS.

6. The number of times cover supervisors employed by the school were used in place of qualified external supply teachers.

7. The number of times HLTAs employed by the school were used in place of qualified external supply teachers.

8. The names of the agencies used to provide external supply staff.

9. The rate that each agency charges the school per employee per day to provide external supply staff.

10. The total amount paid to each agency for the provision of external supply staff.

11. The grand total paid to all agencies for the provision of external supply staff.

12. The due diligence checks carried out by the school on all agencies used to provide external supply staff. By due diligence checks we mean has the school checked that each agency:

• is listed on the National Procurement Framework

• is listed on the Companies House records

• is VAT registered

• is IR35 compliant

• pays staff via PAYE, making appropriate contributions such as National Insurance, holiday pay and pension contributions
