When completing this nomination form please provide as much detail as possible including the impact, notable achievements and overcoming adversity the volunteer has achieved within a sports or physical activity setting. The more information we are given provides us with a better perspective of the great work the nominees are doing therefore giving them more chance of winning.

Please note, where relevant, applicants may be put forward to VAMT (Voluntary Action Merthyr Tydfil) Volunteer Recognition Awards. The award categories are; Sports Volunteer of the Year, Adult Volunteer of the Year, Young Volunteer of the Year and Outstanding Volunteer.
