Children with Disabilities

At Merthyr Tydfil County Borough Council, our Children with Disabilities (CWD) Team supports children and young people with additional needs or disabilities, and their families.

We understand that every child is unique, and our aim is to make sure families get the right help at the right time. We work in a fair, transparent, and supportive way, ensuring that all families feel listened to, respected, and involved in planning the support they receive.

Our team includes experienced social workers and family support staff, who work closely with other professionals such as special educational needs services, Community Occupational Therapists, health professionals, schools, and voluntary organisations. Together, we take a joined-up approach to meet each child’s individual needs.

We support children and young people from birth up to 18 years old, across the whole of the Merthyr Tydfil Borough. The help we offer can include:

  • Assessing the needs of children and families
  • Providing advice, guidance, and advocacy
  • Access to short breaks and respite care
  • Support with adaptations and specialist equipment
  • Referrals to other community or health services
  • Carers’ assessments to understand and support the needs of parents and families

As young people approach adulthood, we also help them prepare for transition, supporting them, their families, and partner agencies to plan for the move from children’s to adult services. Our goal is to make this journey as smooth and positive as possible, ensuring that support continues in a way that meets each young person’s needs and aspirations.

We work in partnership with families every step of the way, making sure they are central to decisions about their child’s care and future.
