Speciality:
This is an established intergrated Community Mental Health Team for Adults and Older adults in Merthyr County Borough Council that has been established to meet the requirements of the citzens of Merthyr.

This post will be responsible for the assessment of people in their own home, communities / hospital/being discharged, identifying the package of support required to meet their needs safely, developing the plan to support this and reviewing whether the plan is being effective in relation to prevention of re-admission.

This post also will support people according to the Mental Health Measure 2010.

Additionally, as the Senior Practitioner in the team this post will provide supervision and daily support to AMHP’s and Social Workers and ensure the effective functioning of the team.

As a member of the leadership team the Senior Practitioner will make decisions with our partners as part of the intergrated CMHT.
