Maintaining your Independence

Home care services could help you with support in your own home with tasks such as support with personal care, which includes washing and dressing. Home care can provide support with specific areas or a range of tasks and can be for a couple of hours a week to full time.

How can I get Home care Services?

If you think that you or someone you know may have a physical disability, hearing or sight difficulties, a terminal illness, are developing confusion/dementia, are frail, are an adult at risk of neglect or abuse, have a learning disability, have a mental health problem, leaving hospital or are a carer and needs a Home Care Service to help you or the person you know to remain at home you will need to contact the Council When you contact the Duty Officer they will explore with you all the difficulties you are facing as well as the ways you currently manage these challenges. They will also want to know what you are hoping to be able to continue to do, or what you would like to be able to achieve. This will be completed in the form of an assessment.

If the outcome of the assessment is that you may need help from home care services, the duty officer will arrange for you to be supported by the Initial Response Service. They may also arrange for you to be supported by the Reablement Service. These services will provide you with support to become more independent in your own home. This support can be provided for a period of up to 6 weeks. However, there is a continuous review period which occurs during this time.

Following Initial Response and Reablement support, if you need a longer term Home Care Service and that you are eligible for care & support, this will be discussed with you. A care & support plan will be written and an assessing and reviewing officer will arrange a package of care with one of our independent providers. or there is an option for you to discuss whether you want to use a Direct Payment to buy the service directly.

Home care services are chargeable, you will be asked to provide your financial details to determine how much you will be required o contribute towards your services. More information can be found on our Paying for Care and Support page (Page is currently being updated).
